Short, simple, and to the point while trying to get an understanding of what the public knows about abortion and how they feel about it.

The cover letter is too little informative. In the question on age, your age intervals overlap. You might want to have an option "I prefer not to say" when asking such sensitive questions as "Do you have a child?" You could've added more question types and formats. Other than that, this was a good attempt to create an Internet survey!
