EVALUATION OF TEAMWORK

This survey is intended to measure evaluations of teamwork after the creative process. It will take about 5 minutes to complete the questionnaire. Please, read carefuly the questions and try provide as honest as possible answers to them. The answers to the questions will remain anonymous and the results will include only discussion of the general trends.
