  1. Your cover letter right away captures the respondent's attention. However, it should have also contained the essential parts of the survey cover letter (esp. information about the researcher). The part that states " this questionnaire was done to find out how many people have actually seen real evidence from reliable sources" is not very clear - how can you make sure the respondents have really seen real evidence? What counts as real evidence? What are reliable sources to you? Do you think they are the same as reliable sources to your respondent? In the question about age, you focus more on the younger segments - why? In the question "How do you feel about Conspiracy theories?" - you should include the answer option that the respondent does not know what consipracy theories are. What does the answer "I watch conspiracy theories only for entertainment." have to do with the question "Do you believe in some Conspiracies?"? Other than that, this was a good attempt to create an Internet survey!
