Since you did not provide a separate question for feedback, I will put it here. Your cover letter is a bit too little informative. If you conduct real research, do not forget to provide the researcher's contacts and more information about the research itself. In the question on age, your age intervals overlap. In the question "Before throwing your clothe away, would you consider doing the following" you could've allowed the respondent to select a couple of options and add his/hers. You could've added more question types and formats. Other than that, this was a good attempt to create an Internet survey!
