INFORMED CONSENT FOR RESEARCH AND AUTHORIZATION FOR THE PROCESSING OF
PERSONAL DATA
Dear Teacher,
We ask you to complete the following questionnaire, proposed within the European Erasmus+ project "Teaching to Be: Supporting Teacher’s Professional Growth and Wellbeing in the Field of Social and Emotional Learning," co-funded by the European Commission. The central theme of the project is the professional well-being of teachers. In addition to the University of Milan-Bicocca (Italy), Lithuania, Latvia, Norway, Portugal, Spain, Austria, and Slovenia are participating in the project.
We invite you to respond to the questions in the most sincere way possible. Data will be collected and analyzed anonymously and in aggregate form to protect the privacy of participants. The processing of personal data, sensitive data, and information collected during the study will be guided by the principles of fairness, legality, transparency, and confidentiality (pursuant to Legislative Decree no. 196 of June 30, 2003, Article 13, as well as the Authorizations of the Data Protection Authority, respectively, no. 2/2014 regarding the processing of data that may reveal health status, particularly, art. 1, paragraph 1.2 letter a) and no. 9/2014 regarding the processing of personal data carried out for scientific research purposes, particularly, arts. 5, 6, 7, 8; art. 7 of Legislative Decree no. 196 of June 30, 2003 and the European Privacy Regulation 679/2016).
Participation in the completion of the questionnaires is voluntary; furthermore, if at any time you change your mind, you can withdraw your consent to participate without having to provide any explanation.
